Em Earlville, uma cidade do centro-oeste, Irma (Jessica Lange) e Roy Applewood (Tom Wilkinson) estão comemorando 25 anos de casados. Roy revela para Irmã e o reverendo Dale Muncie (Randall Arney) que é uma mulher no corpo de um homem e isto está sendo intolerável, assim pretende fazer uma operação para mudança de sexo. Ele passa a usar cada vez mais adereços femininos, com esta atitude sendo vista com intolerância e repúdio pelos colegas de trabalho e membros da sua igreja. Apesar do choque inicial, Irma tenta entendê-lo e apoiá-lo nesta trajetória, que o faz se tornar "Ruth".

After being married to Irma for 25 years, Roy decides the stress of being a woman in a man's body has grown intolerable. He unburdens himself to Irma and their pastor, shocking them both. Roy's decision to pursue a sex-change operation is greeted with intolerance and disgust by some of his co-workers and members of his church. Irma eventually comes to some understanding, and supports Roy through his journey to becoming "Ruth." Written by Ron Kerrigan
